Headquarters, 120th Illinois Volunteers
Memphis, Tennessee

February 25th 1863

I, the undersigned, hereby certify that the Reverend Lewis J. Simpson was regularly elected Chaplain of the 120th Regiment Illinois Volunteer Infantry at an election held in the regimental camp at Vienna, Johnson County, Illinois on the 22nd day of August 1862. And in accordance with this election and upon the recommendation of the undersigned, the said Lewis J. Simpson was appointed and commissioned Chaplain of said 120th Regiment Illinois Volunteer Infantry by His Excellency Richard Yates, Governor of Illinois, which commission bears date October 29th 1862.

I further certify that he has been on duty with and discharged all the duties of Chaplain of the said 120th Regiment Illinois Volunteers from the 22nd August 1862 to the present time.

And further that the said Lewis J. Simpson was not mustered into the service of the United States on the 29th October 1862 as Chaplain. The date of the mustering in of the officers and men of the aforesaid regiment because he was not at that time provided with the requisite certificate from the pastorate of his church whether lost or misplaced. Then received a few days before the day of muster.
