Original Civil War Document, war dated and written in period ink.
Lieutenant Wm F. Dame of the 12th Regt of NH Vols. having applied for a certificate on which to ground an application for leave of absence.
I do hereby certify that I have carefully examined this officer and find he has general nervous debility, caused by a contused wound on the left side over the fifth, sixth & seventh ribs on the 12 day of December last while in front of Fredericksburg by the bursting of a shell, said wound having but recently closed up leaving the lung debilitated also nervous prostration in consequence thereof he is in my opinion unfit for duty. I further declare my belief that he will not be able to resume his duties in a lifetime than thirty days and change of location is necessary to prevent permanent disability or premature death.
